Nope, just a slight case of luck. It happens to us all:

Entei - yellow health, with a Pokeball, no status effects
Rayquaza - 3 rounds (on my first attempt), no status effects.

mewtwo, paralysed, 3 hp, 2 useable moves left, both safeguard, 56 ultra balls, it uses the 2 safeguards and ko's itself with struggle... so i use the master ball on it after having that same scenario happen at least 5 times ^^

mewtwo green health no condition and with only a pokeball!i threw like 499 pokeballs then at the 500 i caught it!

My friend caught a green health (sleeping also) Rayquaza for me, with two ultra balls! I wanted to hug him. ^^
