As far as I know, the ATCM is down at the moment, so there's no link available for it.
As for having a trainer card made, that's what the purpose of the Trainer Cards (http://pokecommunity.com/forumdisplay.php?f=62) forum is. Just ask in any of the active request threads there, and you should be able to find someone who'll make you a card. ^_^
