You get stars by doing the following things:
-Beating the League
-Completing the pokedex (except for event legendaries- Deoxys, Lugia, etc)
-Beating the five contests in all the levels
-Getting all the gold symbols in the Battle Frontier

National, I'm almost positive. If you have any friends with FireRed, LeafGreen, Ruby and Sapphire you can do it no problem. (Remember, some legendaries aren't included.)

Actually, in Emerald, you only need to complete the Hoenn Dex with 200 Pokemon (the 2 excluded ones are Jirachi and Deoxys, the event Pokemon) to get a trainer star. I'm guessing this also applies to Ruby and Sapphire, not just Emerald. I'm not even close to completing the National Dex...

Yeah, the stars are as follows:
* Completing Hoenn Dex (yes, not National)
* Defeating the Pokémon League
* Defeat all contests' Master Ranks
* Getting a 50 win streak at Battle Tower (R/S) / I don't know (E)
And, there is a gossip I've heard, claiming that if you "complete the Trainer Dex", you'll get a fifth star. What does this mean, I don't know. Maybe it has something to do with Trainer's Eyes... Isn't 69 trainers the maximum?
My home forum is Bulbagarden Forums (profile), though I visit this place sometimes too.

The fourth star in E is to get all gold symbols.
And that gossip IS true for R/S, but only true for Jap. E.
If you had an E-Reader and R/S, you could scan extra trainers. The last trainer you scanned would appear in your Trainer's Eyes for a max of 70, getting you the last trainer star. In E, the e-reader system was taken out becuz the system had been discontinued in the US. The e-reader was still there in Jap. Emerald, so you could get the last trainer star.
