Hm, if movies count, then Grave of the Fireflies certainly.

For anime series, I'd say either Air TV or Simoun.

yeah, i have to go with Air...it was one of the few animes that brought me to the brink of tears

I would like to second that as the saddest- as for series... I don't want too many anime series but...
Arc the Lad was pretty sad at parts. Arc the lad anime series

Judging by the responses, I should really watch this AIR TV. Anyway, so far the saddest anime/movie I've seen was 5 Centimeters Per Second. Some might say the ending is more optimistic than you might think but the overall tone was pretty sad. Made me all...hollow inside. (;~;)

yeah, especially with everything that happened to Sara.
Man, I'll never forget that one scene when the soldier comes after her (the one that offers her water) and she goes crazy. I was literally cheering at the end of that.

Spoiler:
the scene afterwards in the desert was so beautiful... the saddest thing that happened to Sara was getting that terrible english VA. But still, more people should watch that show.

Some say he finally moves on and leaves behind his regret of wanting to be with Akira.
