Micro SD Corrupted; Won't Format

A few days when I was trying to manage some of the files on my Micro SD (a 4GB High Capacity model made by pqi) I noticed that the computer would not read it for some reason. Someone on another site suggested that the data on it may be corrupted and I should format it so I could use it again. (Fortunately I decided to back up all the files from it a few days earlier, so I didn't mind having the formatting process delete all the files.)

So, when I tried to use my formatting program, (Panasonic SD Formatter 2.03) It froze. I tried running chkdsk on the drive, but I got an error message saying, "Drive is unreadable or corrupted." I figured that there may be something wrong with the SD Reader, but I used it on my brother's Micro SD, and my computer was able to read the files just fine.

So I ask, Is there any way I can make my Micro SD readable again so I can use it?
