Hands Down, Handbrake. I use this converter all the time and it works wonders. I've been able to convert almost any file and rip DVDs complete with chapter lists, subtitles and alternate audio tracks to the m4v and MP4 formats.
Equipment:

If the source is H.264 video, you can just demux and remux to MP4.

Handbrake is probably the simplest solution, but it's definitely not the best. However, better solutions require a lot of understanding of how video and video containers and such work.
