Why does Johto have it's own gym leaders yet they share the same Pokemon league with Kanto? Since both regions are based off Japan, does that mean Red only became champion of his region, not Japan, if we are to assume the country exists. There has also been mention of Guiana in Gen 1, so are regions countries?

I suppose they are. It takes you a load of effort to get from one country to another in real life, and same could be applied to regions in Pokémon world. Johto and Kanto aren't very distinct (sort of like US and Canada), and so they have bond some partnership of sorts, and leave in peace together. Them sharing the same, exact league could be a part of that deal. As far as champions go, I'm guessing anyone could become it, as long as they're from either of those regions. Same for Elite Four and Gym Leaders.

People seem to all speak the same language in the Pokemon world and that makes it seem like they wouldn't be different countries (unless we mean different countries like England and Scotland are different "countries" according to some people, compared to the UK and France).

Well, Canada, the US, the UK, New Zealand, and Australia are all different countries...

Personally, I've always assumed that Kanto/Johto were a part of the same country, but just different states/provinces/regions, etc. The United Kingdom example could be a good possibility too, as Scotland, England, N. Ireland and Wales are considered seperate countries in some aspects (like sporting competitions), but counted as a single entity otherwise.

I can only assume that the other regions are their own countries though, considering the long distances between them (as far as we know).

Regions is not equal to countries. The first four are based of regions of Japan, so technically not a country. Unova is based of the NY Metropolitan area. Not really that huge. Kalos is based of Northern France. It's still not considered a country since where's Southern France?

So yeah. No. They are not equal.
