That's an Enigma berry. It restores HP if hit by a super effective attack. You could only get it by using GameShark for Generation III games.

Certain Event Pokemon also come with the Enigma berry. The particular one I am thinking of is the 10th Movie Darkrai. At the very least, this means there are legit Enigma berries out there. Though most people wouldn't care much about item-hacking for such things anyway...but such an act would still be against PC's rules. ^^;

i use my AR in legit ways(not saving) and when I hacked a mew once it was holding it, but it wasn't part of the code. So when mew comes around as an event I think it will be holding it
