    Problems I can see:
    - 3 x 200% bug weaknesses and only really forretress to absorb it, be very careful with that
Watch for Heracross who always packs megahorn
Gliscors a good counter with higher speed and aerial ace though I dont know if itd fit on your team

-2 x 400% to ice but you can just switch into vaporeon or forretress

-Substitute seems rather pointless on Torterra because he doesnt take advantage of it in any way, he also is a better physical wall, its always better to work with pokemons strengths

    Consider:

    Torterra (M) @ Leftovers
    Ability: Overgrow
    EVs: 200 HP/252 def/58 atk or speed
    Impish nature (+ def, - sp.atk)
    - Stealth Rock
    - Substitute
    - Leech seed
    - Wood Hammer/Crunch

-I also think itd be better to replace either toxic or yawn on vaporeon with ice beam to get rid of dragons and grasses + again go with Vaporeon's strengths which isnt defense but sp.def

    -Forretress works best like this

    Forretress@Leftovers
    Nature: Relaxed
    EVs: 252 hp,162 atk, 96 def
    -Toxic spikes
    -Spikes/earthquake/gyro ball
    -Rapid Spin
    -Explosion

-Umbreon: Pick either yawn or toxic and moonlight or wish and give it taunt to stop set ups and give possibly give him confuse ray though it worries me that he has no attacking moves

    Torterra (M) @ Leftovers
    Ability: Overgrow
    EVs: 252 HP/152 Atk, 106 Def
    Adament/Impish
    - Stealth Rock
    - Substitute
    - Crunch/Earthquake
    - Wood Hammer
Crunch is on there as a suprise to some ghosts.
No point in backing his special defense, stick with the strong points. Consider EQ for extra STAB
    ---
    Vaporeon (M) @ Leftovers
    Ability: Water Absorb
    EVs: 252 HP/4 SpAtk/252 Def
    Timid nature (+Spd, -Atk)
    - Wish
    - Yawn/Protect
    - Surf
    - Toxic
Yawn if SR is up, otherwise just Toxic to death.
Consider protect to stall out some more toxic pain, also helps self heal with Wish...
    ---
    Forretress (M) @ Leftovers
    Ability: Sturdy
    EVs: 252 HP/6 Atk/252 Def
    Relaxed nature (+Def, -Spd)
    - Explosion
    - Rapid Spin
    - Toxic Spikes/Gyro Ball
    - Spikes
Spin away spikes and blow up their spiker.
Gyro Ball hurts coming from this thing, also Explosion might not be the best thing to do to your spinner as well
    ---
    Azelf @ Expert Belt
    Ability: Levitate
    EVs: 252 Spd/252 SAtk
    Naive nature (+Spd, -SDef)
    - Nasty Plot
    - Psychic
    - Flamethrower
    - Grass Knot
    Standard Nasty Plot Azelf.
Meh, no use for Naive if you aint blowing up in Blissey's face, could change to Timid unless this isn't for Shoddy
    ---
    Umbreon (M) @ Leftovers
    Ability: Synchronize
    EVs: 6 HP/252 Def/252 SDef
    Impish nature (+Def, -SAtk)
    - Wish
    - Moonlight
    - Yawn/Protect
    - Toxic
My other staller, moonlight for additionial survivability.
    ---
    Garchomp (M) @ Life Orb/Salac Berry
    Ability: Sand Veil
    EVs: 4 HP/252 Atk/252 Spd
    Adamant nature (+Atk, -SAtk)
    - Swords Dance
    - Dragon Claw/Outrage
    - Earthquake
    - Fire Fang/Substitute
Life Orb Chomp, what elese is there to say? He handles the steels that otherwise kill my team.
    SubSDChomp = pain

A lot of weird EVs there, SDef shouldn't be an issue for pokemon like Forretress and Torterra as they will get walked over by Flamethrowers and Ice Beams regardless.

Umbreon and Vaporeon seem to be doing the exact same thing, I'd consider replacing one with a more reliable Special Wall... Blissey, Snorlax, Regice, Articuno... whatever you want

The majority of stuff's been covered. I'd also scrap Jolly on torterra, speeding that thing up is pointless. Go Adamant or impish.

Actually, both of them can turn out to be fairly sturdy if they carry wish. Regice doesn't get that sort of reliable recovery. But true, they are doing exactly the same thing. Snorlax would cover the double 4x Ice weakness the team has.

    Suggestion:
    - Azelf (Lead)
    - Garchomp
    - Snorlax
    - Forretress
    - Torterra (If you must)
    - Vaporeon

Azelf as lead is kinda bad, CB Tar can survive any un boosted hit from it and Pursuit it. Same with weavile except its faster and doesnt need to survive. Ive used elf as a lead before and regretted it for that reason.

Starting a match out 5-6 automatically, puts you on the back foot. Azelf is the best lead here however. But i think leading with Chomp may be better.

    Torterra (M) @ Leftovers
    Ability: Overgrow
    EVs: 212 HP / 252 Def / 44 Speed
    Impish nature
    - Stealth Rock
    - Leech Seed
    - Crunch
    - Wood Hammer

Torterra is a tank and should be EV'D as such. Sub just cuts away from its hp needlessly

    ---
    Vaporeon (M) @ Leftovers
    Ability: Water Absorb
    EVs: 188 HP//252 Def / 70 sp.att
    Bold nature
    - Wish
    - Hidden power Electric
    - Surf
    - Ice Beam

Gyarados weak team is Gyarados weak...
    ---
    Forretress (M) @ Leftovers
    Ability: Sturdy
    EVs: 252 HP/ 16 att / 200 def / 40 sp.def
    Relaxed nature (+Def, -Spd)
    - Explosion
    - Rapid Spin
    - Gyro ball
    - Spikes / Toxic Spikes

Ewww, your only hope against weavile is to blow up =/

    ---
    Azelf @ Expert Belt
    Ability: Levitate
    EVs: 42 hp / 252 sp.att / 216 speed
    Timid nature (+Spd, -SDef)
    - Nasty Plot
    - Psychic
    - Flamethrower
    - Grass Knot / Hidden power Fighting

I prefer the ability to hit Heatran and actually do something to dark types.

Max speed is redundant, you outspeed Gengar with 216 evs, 42 hp allows you to survive a weaviles pursuit should you stay in AND a specs starmie surf.

    ---
    Umbreon (M) @ Leftovers
    Ability: Synchronize
    EVs: 6 HP/252 Def/252 SDef
    Impish nature (+Def, -SAtk)
    - Wish
    - Moonlight
    - Yawn
    - Toxic
My other staller, moonlight for additionial survivability.

    Is dead weight
    ---
    Garchomp (M) @ Life Orb
    Ability: Sand Veil
    EVs: 4 HP/252 Atk/252 Spd
    Jolly nature (+Atk, -SAtk)
    - Swords Dance
    - Dragon Claw
    - Earthquake
    - Fire Fang

    Jolly plzz.
    ---

Comments are in bold, IMO you really need a special wall >> Umbreon who is dead weight on this team. Really special sweepers walk all over you. Snorlax as said above or Blissey should suffice.

What you do need aswell is a sold fighting resist, I think Gliscor >> Torterra may be wise, or Heracross, Lucario etc REALLY hurt.

    Something to consider.

Heracross/SD Luke weakness. I would put a standard Gliscor > Torterra, with Stealth Rock as the support move for extra residual damage.

In my opinion Gyro Ball > Toxic Spikes - Gyro Ball allows Forretress to handle Weavile and other stuff while having Explosion as your only attack makes you vulnerable. I actually would suggest Light Screen > Explosion for general support.

For Vaporeon HP Electric > Yawn for Gyarados, I actually would prefer Roar on the last slot instead of Toxic for PHazing, considering that Vaporeon would not actually lure in threatening walls to Toxic them.

Scrap Umbreon - it is easily walled and your team would have no need of the utilities it provides. I would advise you to go with the other rater's suggestions and put Snorlax > Umbreon for special threats, Body Slam/Crunch/Rest/Sleep Talk.

Lastly I believe Yache Berry is a superior option on Garchomp - I am inclined to say that SubSDChomp does not do much unless in Sandstorm scouting for a miss. Including Tyranitar/Hippowdon here would pretty much change the way the team works so I would advise against introducing one of them.
If you do use SubSDChomp, Salac Berry is the best item you can have on it, Brightpowder is not advisable. Leftovers are available if your HP is odd.

Yache Berry is the superior option to break Cresselia which your team would have major problems with.

This team is REALLY Bug weak.

Torterra: Don't give it SDef EVs. It'll still be OHKOed by Ice Beam. Oh, and, give it an Impish Nature.

Vaporeon: Ice Beam>Yawn, DO NOT MAX OUT HP, PUT SOME OF THE HP EVs IN S-ATK

Forry: No S-Def EVs, Max out HP, almost max out Def, put rest in Attack, Impish Nature

Umby: Toxic, Mean Look, Confuse Ray, Baton Pass/Rest/Moonlight/Wish

    Garchomp: Most HP EVs.

Hey, guys, imagine if my Jolly Life Orb Electivire with 3 Elemental Punches and Cross Chop came along...
