A/An differences are phonetic, yes. But I pronounce uterus e|u-d'res, while generally I think it's something like yu-teres. It makes sense that I use "an" because my pronunciation on the "u" does not include a soft 'Y', but more like 'ew', from 'new', making it easier for me to say 'an uterus'. While 'a uterus' sounds awkward in both pronounciations.
