Here is an example of what me and irmck have done:

Banner/Avatar - do you want a banner and/or an avatar?
Size - In pixels width + height
Where do i get image from?- put in URL or image for us to use
Writing- What text do you want on your banner?
Colour Scheme - Which colours do you want on your banner?
