Most epic fail gaming moment?

I was playing Halo 3: ODST's Firefight mode with three other friends. We were trying to get the achievement Vidmaster Challenge: Endure
(In Firefight, on any mission, pass the 4th Set on 4-player Heroic LIVE co-op). For the first couple of waves, we lost 5 of 7 team shared lives. We gathered ourselves gaining up to 29 lives in the waves to come. After Fending off waves of Covenant for over 3 hours on our one and only playthrough, we got passed the 4th set. All we needed to do was go through the 4th set's bonus round and the achievement would be ours. The bonus round starts and ten seconds later the most unbelievable thing happens. The screen go blank for about 2-5 seconds and we were all brought back to the lobby for no reason. We were like "WTF just happened?!", "No (explitive) way!". Haha damn major fail moment.
 
This is not actually a fail. without being big headed I don't think I've failed that bad at a game however.

A few weeks ago I decided to try and beat dead space on impossible mode, to get my platinum trophy (I have ps3)... I beat the game and didn't get the achievement.
I DIDN'T EVEN CHEAT!
Not to mention I came across a weird glitch where another engineer appeared and blocked my path after hammond died...Took an hour to figure out a way to get past him!

I had to replay the whole game mode from the start, it took about 5 hours I rushed and literally ran through levels and I got my 4th platinum trophy.
 
This just happened over the last 30 minutes lol

In WoTMUD I was helping someone EXP, and was bringing them over from Tear to Illian, however on the way we ran into a lion... and then another... and before I knew it, I was dead. Meaning that I had to depend on someone five levels lower than me to guard my corpse while I ran through the forest naked.
 
Hmm.. Prince of Persia and Tomb Raider: Underworld were both a long series of epic failitude for me... Fell to my death many a time (but i did beat them eventually). =/ Um, and I also can't seem to pilot vehicles in space...Star Wars Battlefront II proved that to me. I always get lost and go too far out of bounds; then i get exploded. ='(

edit: Also, when I played through Eternal Poison on ps2 the first time, I stayed up half the night trying to get to the end, only to find.....there is no ending until you go through it the SECOND time. I almost cried.
