Even if this hack is kinda buggy due to Following Pokemon, I'm happy it exists since AFAIK there isn't any other Sinnoh improvement hack that doesn't also add difficulty like Renegade Platinum.

I'm probably not the only one who faced this issue, but it seems the latest patch is broken. Even with a TrashMan ROM as the original post suggested, it still didn't produce a working ROM.
