3rd Gen Dotted Hole

Any walkthrough with the islands included would tell you that you first need to visit Four Island and help Lorelei out.
 
First, you need to help Lorelei out at Four Island in the cave. The rocket scientist will be gone after the mission at Four Island is complete. You then will need to use HM01 Cut to open the door to the Dotted Hole. You will see a bunch of holes in there, now choose the following holes in the same order:

1. Up
2. Left
3. Right
4. Down

After that, you reach what you've been searching for: the Sapphire gem. Before you have time to snatch it, a Team Rocket professor comes into the room and takes it away first. He is the one who tells you the second password: "Yes, nah, Chansey". Now, head back to Five Island and complete the Rocket Warehouse and obtain that Sapphire at the end.
