[Scripting Question] Item-Specific Form-Changing Evolutions

    i remember reading somewhere on the wiki that a supposed evolution idea that could be made is one where the pokemon changes form (sorry for explaining that in a terrible way, my brain is fried right now)
    say for example you have a pokemon (x) that evolves into another pokemon (y). (y) has 2 forms, (y) and (y_1), depending on which evolutionary stone you use on (x).
    i'm trying to figure out how to get this to work.
    i've tried several ways, but i'm a scripting noob. i tried just having it be listed as (y),Item,THUNDERSTONE,(y_1),Item,SUNSTONE, but with that setup, it causes the evolved form to be replaced with the placeholder question mark graphic, and its name is blank. the stats and typing are fine, but the name and graphic aren't, so it half-works.
    could someone please help me? i've been trying to solve this for a week and fear i'll go bald by the end of the month
