Yep, the Hard Mode patch uses the Beta Starters. Unfortunately, I couldn't figure out where that specific text is for Green's "choose Pokemon" dialoge, so that's a known issue. Pretty sure...
It's basically the same as vanilla, you get it from Oak after beating the game. The main difference is I've reduced the number of caught Pokemon required from 60 down to 30. Keep in mind, these need to be Kanto originals to count.

I'm gonna guess you're not on Version 1.2. I thought this might be an issue with the map state, like the map loading differently if the SS Anne is there or not. But I just playtested it and the items show up both before and after the SS Anne leaves. Unless there is some weird emulator issue on...
Not really. There is the one major one, being Bill's Garden. There are also a few small areas added here and there to accommodate some of the Legendaries, but they're mostly just a short path and then a room with the Pokémon. Outside of that, it's vanilla Kanto.

I've updated the v1.2 download link with a fixed patch. It should now learn Cut correctly.

In that case, I'd recommend using the "Walk through walls" cheat once you get Surf. If you surf around the guy at the gate, you can still get into the SS Anne harbor after the ship has left. I've confirmed that this cheat works:
