    Ralts...
    Feebas...
    Ditto...

This is why we need a Simple Questions Thread. XD

Ralts and Ditto can only be found by using the PokeRadar.
Ralts in the South of Routes 203 & 204 and Ditto on Route 218
Feebas is located on only four squares which rotate and change every day and is in Mt. Coronet (Fish w/ Super Rod will help)

No, it's kinda random. You can't actually tell which Pokemon is which unfortunately. All you get told is that there is a Pokemon there. The grass wiggles... and apparently you can tell if it's shiny or not by that. But the chances of it being shiny is still very slim.

OMG!!! A few times I've seen a shiny wiggley grass thing...haha...Do you know where to get Miltank?

It may look shiny, but apparently it's suppose to do more than sparkle a little. I've tried it lots before.

As for Miltank, also PokeRadar, Routes 209 & 210-South

You can catch anything with the radar, but those ones you asked (National Dex/Kanto/Johto/Hoenn region Pokemon) are harder to find, and therefore classified as "rare" . And in Feebas's case, "very rare". It'll take some luck and patience to find them.

Use the Honey thingy. I dont know what it's called in English (as I play the Japanese version) but once you have put the honey on the tree and waited for around 8-12 or so hours, you'll get a Pokemon there. Munchlax is also classified as "rare" as it can only be found there in the Valley and on that tree specifically. You'll also get other Pokemon on that tree via the Honey method, so dont just expect a Munchlax straight away! I havent got one by this method yet.
