More traditional RPG Maker text box format (character avatars)?

Hi! I was wondering if anyone knows how to make the text box format include character avatars like in default RPG Maker. I know you can display an image (as well as a cropped square image) above the text box, but I was just wondering if you can display one in the interior of the box like in traditional RPGs. Thanks! =D

Edit: On a related note, if there were a way to place an image above the text box but outside the border, like in that old fangame Generation Zero, that could also work for what I want, if there's no gap between the box and the image.
Here's an example from Generation Zero:

    Thanks again!
