I'm wondering if it's possible to make an ability that can set up Substitute with that specific Pokemon's alternate form graphic.
How the ability looks in the abilities.txt:

Yes. This is possible. What you described is basically Mimikyu's Disguise ability which activates when its health is at 50%. Use that script and make changes accordingly
