Houndoom: Unfortunantly, the form that you copied is not offical. Even though the original poster said it was for TCard 4.5, it was only for 4. Please click the first link in my sig for all the TCard offerings. I shall begin to make this one, but if you want the entire 4.5 experience, you will need to include additional information.

Potential Requesters: Please, Please, click the first link in my sig. This takes you to the forms post, which shows all the cards I'm offering. Copying posts other people have made and using them, results in more work for me (I have to decifer the different form), and potentially, more work for you (the form may be incomplete).
