Double post, in one post.

This is normal. It is a feature of a mod that is installed on the forum that automatically merges double posts for users.

What I find a bit odd is that sometimes the two posts merge, but other times it comes out as a literal double post. What makes the difference?

It seems to be more of a problem when you use Quick Reply over Advanced Reply. I always use Advanced Reply and never get a double post, but others who tend to use Quick Reply get double posts.

The increase also has something to do with PC's new server. What that issue is, I don't know. I do know that once threads hit a high post amount, the rate of double posting in them increases. At some point, some threads will have to be remade to avoid this double posting issue.
