I don't think May's last name has ever been mentioned in the anime, but most people use Maple. I think May Maple has a very nice ring to it myself :)

And as for May's outfit...I think she looks great in her current outfit but it'd be neat to see her in the outfit she has in Pokemon Emerald >.>
