Any Ashford Academy episode in Code Geass: Lelouch of the Rebellion or Code Geass R2: Lelouch of the Rebellion. The attempted humor in those episodes were worthy of "WTF" comments.

Nothing really strikes me as wtf (aside from why ecchi is so commonplace as to be in every anime ever) so I suppose the above is the closest thing, but it's pretty much par for the course if you've read the previous 184 pages.

Anything that is from the One Piece anime that isn't in a battle. Heck nevermind...Some of those moments happen there.
I pull something from a recent episode that isn't cannon. The last five minutes of the Special Historical Arc in the second episode of the arc. Yeah...If you saw it then you know what I am talking about. I saw it coming and I knew something like that was going to happen.

This I definitely agree with.
There are probably others I can think of - there's practically at least one in every anime - but the most recent one that came up would be WTF @ Kadokawa/Endless Eight in SHnY.
