In generations iii and iv, your trainer card has stars on it to indicate how many of the in-game achievements you've completed. In Gens III, it's 4 stars, and in generation IV, it's five. In Gen IV, if you have 4 or more trainer stars, the Pokemon Nurses will address you by name. While this is a very small detail, it's interesting to note.

In generation III, you get trainer stars for:

-Getting into the Hall of Fame (both FR/LG and R/S/E)
    -Completing the Regional Pokedex (FR/LG & R/S/E)
-Completing the National Pokedex (FR/LG only)
-Getting Five Paintings in the Lilycove Museum (R/S/E only)
-Winning 50 battles in a row in the Battle Tower (R/S only)
-Collect 200 Berries in Dodrio Berry Picking and Jump 200 times in Pokemon Jump (FR/LG only)
-Get all the Gold Symbols in the Battle Frontier (obviously Emerald Only).

    As for generation IV:

-Get in the hall of fame (All gen IV main series games)
-Complete the National Pokedex (All gen IV games)
-Win a Master Rank Contest (D/P/PT only)
-Beat all 10 records in the Pokeathlon (HG/SS only)
-Get a Platinum Flag in your Underground Secret Base (D/P/PT only)
-Get a 100 battle win streak in the Battle Tower (all gen IV games)
-Get a Leaf Crown on one of your Pokemon (HG/SS only)

How many stars do your trainer cards have?

in Sapphire and Emerald, I believe I have two.
In HeartGold, and I have a 4 star trainer card, and it was a very lengthy effort, if I do say so myself, it took a really long time.
I'm pretty sure in my D/P/PT games, I have 2 or maybe three in Platinum, although idk about that, I haven't checked in awhile.
