Its been daunting in my head:

Is Gengar a ghost image of Clefable?
Its strikingly similar. They both have three fingers, three toes, and the same round shape.
The curl on Clefable's head has turned into spikes on Gengar, and the wings on Clefable's back turn into spikes on Gengars back.
Pokémon sure has its creepy parts.
Theres other ones, like Gastly and Cloyster. Its weird. O.o

I've heard that too. Maybe it was part of the inspiration for Gengar's design, to be a dark shadow of a fairy pokémon. But I don't think any official sources confirm this today.

Yeah it was either meant to be like that or its a complete coincidence. Or they wanted to show the dark side of a cute fairy. It is sinister though.

I think a more likely explanation is this: GameFreak simply recycled the same design for two Pokemon.

It looks viable, but really I'm guessing GameFreak will probably never confirm it so for me it's up for anyone to decide upon themselves whether they think they're related or not.
