That one mission in Jedi Outcast, where you're supposed to stealth your way through it, even though on the way through, you can (laughably) wipe out everyone with ease...although there are a few areas where there's a guy at the alarm already...

One thing I do hate is when the game is entirely set up around party combat but it suddenly throws you into situations where you have to fight large battles alone.

Depends on the game. In games that have terrible stealth mechanics, sure, but in games that do Stealth well, like Dishonored, The Elder Scrolls Series, and Metal Gear, they're pretty fun. Same for games that aren't stealth-based but do Stealth surprisingly well (Like Wind Waker).

Yeah, generally when a game gives me the choice of killing as few- if any- people as possible, I will. Almost intuitively. I think I went my whole Deus Ex playthrough without killing a single person.
