Pre-requirements:
- Having a good eye for effects and colours and all that
- Being good with your mouse

1st step: Get Photoshop or GIMP or whatever.
2nd step: Mess around with the program and images for a few days.
3rd step: Once you have an idea of what does what, focus on the effects you like more and start practicing.

That's what I did to learn how to do my own themes.
