My own list includes...
» Onions. I think it's the texture of them, because I don't mind the flavor and I actually really like onion rings. But regular, non-fried non-breaded onions? Yeah, might as well forget trying to get me to eat them. I will actually try my best to pick them out of foods that contain them. There'll be a pile of onions on my plate when I'm done eating.
» Bell peppers. But only green ones. I think they're just too bitter for my taste. I don't mind any other kinds of bell peppers. Not my favorite things, but I won't snub them.
» Other things include olives and mushrooms. Do not like the taste. At all.
I can't really say who introduced me to these foods - most likely my parents, since I've had my dislikes of them for as long as I can remember. I feel like I'm missing some, but it's probably a food I rarely run into anyway.
...Oh right, I remember one. A more recent one. I tried liver because I saw it at a buffet and I was like "huh, never had this, I'll try it out". Never again. That was the first time in absolute ages that a food has triggered my gag reflex so badly.
