Yes. This did happen once.

Animation errors. The anime has plenty. Either the animators are lazy, and don't go back to fix them, or just didn't notice them during production. What are some of the most ridiculous or noticeable animation errors you've seen in the Pokemon anime? Bulbapedia has a gallery of them, so that'd be easier on you.
 
Me and some friends started rewatching Indigo Plateau about a two years ago. Got this gem where Misty gets kidnapped by Team Rocket and loses absolutely loses her head.

A bonus, because it's not...really...an error, just hilariously drawn. Then again, given the context, it was probably both.

In Johto's Igglybuff filler there was a shot of Jigglypuff behind the COTD girl in one scene despite the fact that TR had snatched said Jigglypuff seconds beforehand.


I swear, this is my exact expression every time we get a TR-centric episode.
 
The following might come across strange, so you're forewarned!

I've seen my fair share of animation flubs over time. However, there's only two that I can think of right away:
*In Pi-Kahuna, when Ash was surfing, and ended up getting cramps in his foot, the close-up shot of his feet shows that he has two left feet.
*In A Dairy Tale Ending, after Ash gets kicked in the crotch by Whitney's Miltank, when Milton explains about the grabbing by the tail, Whitney's socks are missing.
 
There's a green Weedle in the background in one episode. I think it was in the episode where Todd first appears.

I can't remember which episode it was (I think it was a Silver Conference episode), but Prof. Oak's eyebrows change colour.

In one shot during the first movie, Rapidash's fire mane is missing.
 
I remember there was a screenshot of a Breloom and Marill that were miscoloured. Couldn't exactly remember the episode though.

That would be Weekend Warrior, which was an episode during the Battle Frontier saga. Coloring errors are quite common in the anime, it is probably the most common animation error I've seen. Miscoloring is kinda lazy, if you ask me, but ah well, aha.
