Well...it's a tablet. So it shouldn't be any bigger than 500 mg, which isn't that big compared to the rest of the stuff that leaves your body. Assuming it's a tablet by medical standards...or whatever...

Unfortunately it has to be that way; if you try to disgorge the charcoal after it's absorbed the poison, you run the risk of inhaling it into your lungs.
