It's supposed to mean that a person hold genuine affection for you, care's about your well-being/happiness and/or wants you to be a part of their life. What it usually means is "I think you're kind of cool but wouldn't really be that fussed if you weren't here".

Well, that's what I get for trying to sound all edgy.. xD

I'll give you an example. Say there's a couple, alright? One person in the relationship is passive and is afraid of losing the other. The other person pretty much dominates and is a complete sociopath who guilt trips and makes the other person feel bad whenever they get into a disagreement.
Passive Person: "Y-You really shouldn't be using my expensive dress shirt as toilet paper.."
Guilt Tripper: "Why are you doing this to me? I-I love you.."
Passive Person: "I'm sorry! It's okay! I didn't mean it!"

If I hear this from someone, I assume that it's unconditional and that they would be putting their life on the line for me. Granted, I've only really heard this from my parents, but I would never say this to anyone that I didn't feel this way about.
